  • Schedule your free 15 minute introduction call with your personal doctor, Dr. Cindy Dafashy, to learn more about membership. If you decide to enroll, we will then schedule your first appointment.

    New patient appointments are scheduled for 1 hour, giving you time to dive deep into your needs.

  • Pricing:

    • Adult membership is $100/month

    • Child membership is $50/month, requires 1 adult membership

    • One-time enrollment fee $150

    • No cancellation fees ever

    • Discounts available for family members

    • Small business employer plans available

  • With direct access, extended appointment visits, offered in office and virtually, you can create a true lasting relationship with your doctor to develop personalized health and wellness plans, designed around you.
